NAC432A.510. Documents to be submitted by parent who resides outside county.

  •      1. If the operator of a facility conditionally admits a child whose parent has not established permanent residence in the county in which the facility is located, the operator shall keep the proof submitted by the parent that he or she has not established a permanent residence in the county on file at the facility for 1 year. The proof must be in the form designated by the licensing authority and may include a driver’s license.

         2. In any case governed by this section, the parent shall submit a written statement of the immunization status of the child to the operator of the facility. The statement must be signed by the parent.
