The Division will impose at least one sanction upon any intermediary service organization that has a violation with a severity level of four or a severity and scope score of six or more as determined pursuant to the provisions of NAC 449.39551 to 449.39555, inclusive. More than one of these sanctions may be imposed at the discretion of the Division.
