NAC459.95337. Certification of required documents.

     1. Any document required to be submitted pursuant to NAC 459.952 to 459.95528, inclusive, that is required to be certified must contain language for certification that substantially conforms to one of the following forms:

     (a) I certify under penalty of law that the information provided in this document is true, accurate and complete. I am aware that there are significant civil and criminal penalties for submitting false, inaccurate or incomplete information.

     (b) I certify under penalty of law that I have personally examined and am familiar with the information submitted in this document and all attached documents and that, based on my inquiry of the natural persons immediately responsible for obtaining the information, I believe that the submitted information is true, accurate and complete.      2. The certification must be signed by the sole proprietor of the facility, the highest ranking corporate officer of the facility, a partner at the facility, the manager of the facility or a person designated by one of those persons to sign the certification.
