NAC488.125. Certificates of number: Duplicates.

If a certificate of number is lost or destroyed, the person whose name appears on the certificate as the owner may apply for a duplicate certificate by submitting to the Department:

1. An application on a form provided by the Department; and

2. A fee of $20.
