Latest version.

Each certificate of completion must include:

     1. The name, address and number of the student’s license issued by the Division;

     2. The name of the sponsor;

     3. The name of the instructor of the course;

     4. The name and number of the course;

     5. The number of hours of instruction completed; and

     6. The date the course was completed.
