NAC555.371. Renewal of license for pest control business: Annual report.

In addition to the information required by NAC 555.350, each application for the renewal of a license for a pest control business must include an annual report of all the pesticides applied by the pest control business during the immediately preceding 12 months. The report must be on a form provided by the Director and must include, without limitation:

     1. The brand name of each pesticide that was applied;

     2. The registration number assigned to the pesticide by the Environmental Protection Agency; and

     3. A statement indicating whether the pesticide that was applied is a restricted-use pesticide.
