Each page of a statement of costs must show the period covered by the information on the page. The following paragraph must appear on each statement of cost above the signature of the distributor or his or her accountant:

I hereby certify that this statement of costs is based upon the records kept in the ordinary course of this business operation for the period shown and is correct to the best of my knowledge.
