NAC617.100. Examining physician to provide copies of forms.

The examining physician who conducts a physical examination pursuant to NRS 617.455 or 617.457 shall provide the employer and the employee with a copy of:

1. The form required to be completed by subsection 4 of NAC 617.075; and

2. Each of the forms required to be completed by NAC 617.050 or 617.070,

following the physical examination.
