  • Schedule F-2 is a schedule that shows the weighted average cost of preferred stock capital based upon the following data for each class and series of preferred stock outstanding according to the balance sheet as of the end of the period of testing and the cost of new securities issued pursuant to subsection 3 of NRS 704.110:

         1. Title;

         2. Date of issuance;

         3. If callable, the call price;

         4. If convertible, the terms of conversion;

         5. Dividend rate;

         6. Par or stated amount of the issue, gross proceeds and any premiums;

         7. The amount and percentage of gross proceeds of the issuance expense, including underwriters’ discounts or commissions;

         8. Net proceeds;

         9. Cost of the money;

         10. Whether the issue was offered to stockholders through subscription rights or to the public; and

         11. If the issue is owned by an affiliate, the name and relationship of the owner.
