Tests to demonstrate mechanical integrity must be conducted at least once each 5 years for the life of an injection well. The Director may require such tests more frequently if conditions of the operation so warrant. The holder of a permit shall notify the Director at least 45 days before the date upon which a test for mechanical integrity is to be performed.
